    Case Officer - OR Tambo

    Purpose of the position:

    • Responsible for implementing and monitoring the Case Management Model (CMM) through adherence support for ART patients until they are eligible for decanting to any of the differentiated models of care.

    Minimum Requirements    

    • Grade 12 certificate
    • Enrolled nurse qualification will be an added advantage (most suitable for pediatric streams)
    • At least 2 years’ relevant experience in counselling and testing
    • Adherence counselling
    • Basic HIV testing and counselling
    • Index case trailing
    • Able to communicate in English and isiXhosa is an advantage

    Duties and Responsibilities    

    • Capture details of clients on ART for less than one year and newly initiated patients on ART on the CMM excel spreadsheet, and plot all due appointments
    • Capture clients with high viral loads, pediatric clients, pre-ART clients and pregnant women
    • Phone clients and conducts daily linkage to a CHW or any appropriate community person for those who missed their appointment; pay a visit to clients who missed their appointments, patients who are not virally suppressed, and patients needing additional care
    • Issue routine reminders according to the schedule of appointments by sending SMS reminders
    • Responding to messages from clients and crafting an appropriate and professional response
    • Refer patients for enrolment in support groups according to the patient’s preference

    Social Auxiliary Worker, PPPREV_Zululand

    Purpose of the position: 

    • This post will provide psychosocial services under the supervision of a social worker within the context of an HIV Priority Populations Prevention programme focusing on adolescent girls and young women (AGYW) and Adolescent boys and young men (ABYM). 

    Minimum Requirements    

    • Grade 12 or equivalent  
    • Certificate in Social Auxiliary Work from a recognized college or institution
    • Two years training; or equivalent combination of education and experience.
    • Minimum of 2 years relevant working experience
    • Must be registered with SACSSP (South African Council for Social Service Professions)
    • Proficiency in English and isiZulu languages
    • Computer literate in MS office advantageous
    • Clear criminal record
    • Valid code 8 driver’s license 

    Duties and Responsibilities    

    • Conducts comprehensive psycho-social assessments of AGYW and their families  
    • Conducts House hold profiling using profiling tools 
    • Provide minimum package of community-based care and refer to social worker /facility for continuum of care 
    • Supports Social Workers in assessing the needs of AGYW & ABYM and parent’s/care givers and guardians 
    • Provides structural programmes including SASA  
    • Networks with all relevant stakeholders  
    • Assists with the identification of advocacy issues

    HIV Ambassador

    Job Description

    • The core responsibility of the HIV Ambassador is community mobilisation and demand creation towards the achievement of the UNAIDS 95-95-95 targets. The approach entails interfacing with proximal facilities to get a sense of their priority programme activities, then embarking on awareness campaigns and social activations to promote and advocate for those priority programmes. Furthermore, the HIV Ambassador would interact with key influencers in the communities (youth formations, traditional leaders, ward counselors) to create demand for health services. Additionally, the HIV Ambassador would assist the facilities in the tracing of patients on missed appointments lists in communities around their places of stay. The target population includes: ART & TB clients who have missed their appointments, mother-baby pair, unsuppressed viral loads and elicit index client information for follow up and pregnant women.

    Minimum Requirements    

    • Minimum of grade 10 certificate 
    • Living openly with HIV 
    • A community advocate on health issues especially HIV/AIDS 
    • Able to communicate in English and isiXhosa is an advantage 
    • At least two (2) years’ relevant experience in counselling and testing 
    • Basic HIV counselling and testing

    Duties and Responsibilities    

    • Conduct health education in communities on HIV, AIDS & TB as part of the 95-95-95 strategy 
    • Source missed appointment list from Data Capturer for daily tracing 
    • Create health awareness campaigns in identified communities on the importance of 95-95-95 status for HIV and TB, stigma and discrimination and on treatment adherence
    • Leverage social platforms (imbizo, youth gatherings, LAC, DAC etc.) to promote 
    • Refer patients for enrolment in support groups according to the patient’s preference 
    • Work as part of a multi-disciplinary team to optimize targets

    • Provides administrative support in terms of daily reports and report writing.

    Enrolled Nurse (Paeds) - Chris Hani

    Purpose of the position:

    • Provides clinical and administrative duties including but not limited to; supervision of case officers, linkage of clients into care, supports case management solutions for adults and pediatrics in attainment of 2nd 95 and 3rd 95. Liaise with various stakeholders, district, health facilities and other government departments.

    Minimum Requirements    

    • Grade 12 plus Enrolled Nurse qualification
    • HIV and TB management trainings, WBPHCOT will be an added advantage
    • A minimum of 2 years’ experience in HIV and TB programs
    • Registration with the South African Nursing Council as an enrolled nurse

    Duties and Responsibilities    

    • Blood monitoring and collection as per required monitoring intervals
    • Be familiar with the ROAD TO HEALTH BOOKLET and ensure completion of all relevant section at each visit working closely with P/N
    • Align EPI visits with HIV management of children and adolescents including growth monitoring and development
    • Key entry point screening and testing of children and adolescents 0-19 years for HIV/TB/Covid-19 as per HIV Matrix of Intervention and HIV/TB/PMTCT guidelines, including Antenatal/Post-natal/Breastfeeding clients using IMCI Checklist/PATEST tool/HIV adult screening tool, handholding those tested positive and link them to care
    • Enrol all children and adolescents newly diagnosed with HIV on case management model, for continuity of care and support and facilitate decanting to Differentiated Models of care as per SOP, working closely with community teams and OVC partners
    • Provide preventative counselling for women tested HIV negative during ANC/breastfeeding/PNC and offer Pre-Exposure Prophylaxis (PrEP).
    • Offer index testing services for ANC/breastfeeding/PNC partners and provide HIVSS test kits where necessary
    • Monitor child development and immunisation status of all children <5 years irrespective of HIV exposure history
    • Periodically, conduct MCH clinical file audit, to ensure effective implementation of PMTCT guidelines and effective clinical management of clients.
    • Ensures that all children and adolescents 0-19 years are in optimum user-friendly formularies and regimen as per age & weight, to enhance adherence and VL suppression
    • Take VL blood as scheduled in PMTCT guidelines and enrolled all children and adolescents with VL >50 c/ml on enhanced adherence counselling (EAC), report any issues needing attention to P/N or VL champion
    • Weekly actioning of RFA list as appear on RFA dashboard/ elabs device.
    • When due refer all clients require psychosocial support, disclosure and adherence counselling to social worker via FTL/PN.
    • Facilitate decanting of eligible < 19 years clients and periodically evaluate clients due for script renewal, with assistance of FTL/PN
    • Support establishment of youth-zones and its activities to reach <19 years whom consulted on that youth-friendly corner
    • Work with social workers to establish youth clubs, Post Natal clubs, Viremia clubs and Family care days.
    • Participate in Operation Phuthuma Nerve Centre activities that contributes to MCH indicators, including MCH score card, where applicable
