    Community Health Worker – Home Based Care

    Anova is an NGO that empowers people and changes lives. Good health and quality of life is what motivates us to provide healthcare solutions and support for those who need it most.

    We are looking for Community Health Workers who will offer an integrated health and wellness service package to the individuals in the households being visited. The package consists of interventions that focus on health and wellness (ranging from prevention, promotion, rehabilitation and palliation).

    Key Duties and Responsibilities

    Physical Care

    • Assist the patient with personal hygiene.
    • Wound care, how to dress a wound using appropriate material.
    • Stoma therapy, catheter care, nail and foot care, tracheotomy care.
    • Observations: temperature pulse and respiration.
    • Be able to assess an emergency situation and provide basic life support and first aid in order to stabilize a patient.
    • Assist patient with mobility, exercise limbs, take patient for walks, assist in and out of bed and to the toilet, have knowledge of motion exercises.
    • Preparation of patient's snacks and meals if necessary.
    • Distribution of food parcels where necessary and if patient's circumstances meet the required criteria.

     Care of Patient’s Environment

    • Make patients bed, replacing with clean bed linen when necessary. 
    • Tidy and clean the patients’ sleeping quarters, disposing of any soiled night clothes and bed linen and washing of these if necessary. 
    • Infection control. 

     Health and Education

    • Educate patient and relatives about caring for the patient at home and resources available.
    • Specialized care of catheters etc.
    • Universal blood precautions.
    • Nutrition: Identify food groups and educate patient/family on planning suitable meals for their needs 

     Referral Criteria

    • Refer patients to health facilities and service providers where necessary.
    • Know the when, why and how of referrals.
    • Reporting of deterioration of patient’s condition to supervisor

     Administration & General Duties

    • Establish a set of records and ensure that they are up to date.
    • Complete the appropriate record forms accurately and promptly ensuring that these are given to the supervisor timeously.
    • Follow all policies and procedures as required by the organization and ensure that all work targets are met.
    • Follow daily work schedule as worked out by the supervisor.
    • Participate in meetings as required as well as attending training courses and workshops related to the nature of the work done.

     Medication

    • Supervision/administration of medication.
    • Collection of prescribed medication (repeat) from the CHC
    • Ensure as far as possible that the patient completes their treatment in the prescribed time given.
    • Report any patient that defaults on treatment immediately to the staff of the CHC.
    • Report any reaction to any medication to the OTL.

     Any other tasks as agreed with line manager.

    Minimum Qualifications, Experience and Skills Required 

    • Grade 10-12
    • Homebased carer qualification/certificate

    • 10-day HIV Counselling Certificate (advantageous)

    Outreach Team Leader: Enrolled Nurse (X2)

    We are looking for an energetic and dynamic Outreach Team Leader: Enrolled Nurse to lead the Home-Based Carers, CDU, CDL, Rehab Adherence, CDU Staff.

    Key Duties and Responsibilities:

    • Design weekly work plans for the CHW and for him or herself (home visits, household assessments,
    • community outreach events, etc.)
    • Manage the Community Healthcare Workers per node.
    • Work closely with the DoH facility management and the community leaders to facilitate the Community-Oriented Primary Care approach – meaning the OTL is required to attend the local community meetings.
    • Attend and actively participate the multi-disciplinary meetings organised by the DoH facility management.
    • Actively participate in the wellness and prevention interventions, e.g., administering of vaccines, screening for non-communicable diseases, family planning, etc.
    • Plan and implement mentorship and coaching sessions for the CHW on the Integrated Health and Wellness Service package.
    • Routinely record/update patient files and stats.
    • Attend training sessions arranged by the DoH and/or Anova.
    • Routinely travel to dedicated Anova, wellness outreach venues and/or DOH facilities in the Cape Metropole District, when required.

    Senior Technical Advisor: Paediatric & Adolescent Care

    The successful candidate oversees and manage Anova’s paediatric & adolescent HIV Care & Treatment programme and initiatives in the district; to achieve the 95-95-95 targets. This includes programme implementation and review, senior medical expertise, psychosocial support, quality improvement initiatives and stakeholder management. 


    Key Duties and Responsibilities

    Programme Management & Technical Assistance

    • Oversee implementation of 959595 HIV/TB strategy for children & adolescents (0 – 19 years) across JHB sub-districts according to workplans, including scale-up of high yield strategies to improve case finding, linkage to care and support enhanced adherence to ART and viral suppression for children & adolescents living with HIV.
    • Conduct data analysis and review of programme performance against targets/programme outputs. Support sub-district technical teams to develop Quality Improvement plans for poor performance areas.
    • Oversee the development and provision of paediatrics/adolescents training to Anova and DoH multidisciplinary staff, and the development of guidelines, policies, and protocols.
    • Support the paediatrics/adolescents team with clinical requirements for community-based service delivery.
    • Support sub-districts to write up best practice case/reports and participate in relevant research projects in the district accordingly, to keep abreast of issues and add to the pool of knowledge within Anova.

    Health System Strengthening & Programme Quality Improvement

    • Support Anova paediatrics/adolescent TAs (through mentorship, providing technical advice, conducting joint stakeholder engagement activities, conducting facility level support visits, etc.) to ensure that agreed upon activities are successfully implemented in facilities.
    • Identify risks to successful implementation of 959595 activities, address it with relevant teams (e.g., Anova, District management, etc.) and put measures in place to mitigate the risk.
    • Facilitate improvement in early infant diagnosis and ART initiation in children across the district. 
    • Oversee processes that improve Linkage to care and ART initiation in children including audits on the NHLS RfA reports and Tier.net Pre-ART and ART modules to ensure that HIV positive infants and children are initiated on ART. (in conjunction with Seconded TA)
    • Support sub-district teams to monitor and institute improvement interventions for VL completion and suppression.
    • Work with sub-district TAs to improve on NAS implementation, including DMOC, PSS and the support and management of OVC’s affected by HIV and Children with unsuppressed viral loads.
    • Oversee activities that identify and improve on gaps on all the TB related 3-90s, including IPT initiation & completion and Case Finding strategies.
    • Enable implementation of paediatric/adolescent ACC support at identified facilities, through TAs and ACC MOs
    • Conduct Paediatric/Adolescent programme quality assessments using evaluation and audit tools, such as file audit tools. Guide sub-district TAs to implement QI activities to demonstrate improvement of poor performing areas.
    • Co-ordinate paediatric/adolescent donor and/or Anova management visits together with sub-district managers. Be available for such facility or DoH engagement visits.

    Capacity Building

    • Coordinate and support sub-district TAs to conduct capacity building across the clusters, and ensure monitoring, reporting and accountability systems are in place, working with District Training team (DoH and Anova)
    • Use different data sources to identify training needs and ensure capacity building takes place according to these.
    • Provide training & mentorship to relevant staff (internal & external staff) on key issues related to Management of children and Adolescents living with HIV.
    • Conduct PMTCT/VTP, NIMART, Basic TB and any other HIV/TB/PMTCT and related trainings that supports the current needs of the department of health as the need arises.

    Team Management, Coaching and Supervision

    • Provide clinical training and mentorship on specific topics and oversee implementation of such activities by TAs and other clinicians.
    • Capacitate Anova staff with relevant skills to conduct paediatric HIV support activities.
    • Manage the day-to-day activities of the Paeds and Adolescent TAS and provide mentoring and coaching to paeds and adolescent team.
    • Be familiar with Anova HR and recruitment policies and work with HR on recruitment in line with such policies.
    • Identify training needs and opportunities for programme staff and arrange for such trainings.
    • Ensure that staff performance Contracts linked to the strategic plan of the organisation.
    • Execute performance reviews for paeds/adol TAs bi-annually or as directed by HR.

    Stakeholder engagement

    • Establish, manage, and maintain working relationships with sub-districts, facilities and other partner stakeholders for successful implementation of DOH and Anova workplans
    • Attend monthly and adhoc meetings with supported sub-districts, district, and partners.
    • Liaise with relevant managers of the Department of Health to ensure maximum buy-in and partnership, including engagement at a sub-district level.
    • Work closely with other partners supporting Paeds & Adolescent programmes within the district to share best practices and align activities.
    • Support TAs in their sub-district level and facility programme planning, review and other meetings.

    Reporting and Other Activities

    • Compile ad hoc, monthly, quarterly report to line manager, funders and DOH as required.

    Essential Qualifications and Experience

    • MBChB degree or BCurr with a Postgraduate qualification with Primary Health Care speciality (Nurse Clinician)
    • Valid HPCSA registration as Independent Practitioner or valid SANC registration
    • Minimum 2 years’ post community service clinical experience (for medical doctors) and 5 years for Nurse Clinicians
    • Paediatric and Adolescent HIV/TB Management training and experience essential.
    • NIMART trained and graduated from the mentorship programme (Nurse Clinician)
    • Quality Improvement and project and team management skills
    • Experience in training an advantage.

    Monitoring And Evaluation Officer

    Technical Advisor: Paediatric & Adolescent Care (District Secondment)

    To support Johannesburg district HAST team to improve paediatric & adolescent HIV and TB Care & Treatment programme implementation and monitoring in the district; in order to achieve the 95-95-95 targets. Engage with the various stakeholders involved in care of children and adolescents to improve client reach and access to available resources.
     

    Key Duties and Responsibilities

    Programme Management & Technical Assistance

    • Play an active role within the Johannesburg District team, working with relevant District managers, to support the District to oversee implementation of 959595 HIV/TB strategy for children & adolescents (0 – 19 years) according to workplans, including scale-up of high yield strategies to improve case finding, linkage to care and support enhanced adherence to ART and viral suppression for children & adolescents living with HIV.
    • Work with District PMER team to undertake monthly/quarterly programme performance review, and identify areas for improvement in data and performance, and areas where additional data is needed to monitor the programme, advocating for its availability.
    • Work with District team to strengthen Matrix of Interventions implementation, monitoring and reporting at facility, sub-district and support process or sharing with provincial management.
    • Support District with compilation of best practices/case reports of work being done in the District, for learning and sharing with other districts/provinces.
    • Work with DoH and Anova facility/sub-district/district teams to identify tools and strategies for improvement and advocate for the review for adoption of these by District and sharing with province. Once adopted, oversee implementation with the District team through DoH sub-district leads.
    • Oversee DoH processes that improve Linkage to care and ART initiation in children including uptake of NHLS Results for Action, audits on the RfA reports and Tier.net Pre-ART and ART modules to ensure that HIV positive infants and children are initiated on ART 
    • Work with Integrated School Health Programme to increase services available from existing resources (e.g., Partner outreach teams, DoH funded NPOs)
    • Working with District and Partner community teams, determine potential areas to leverage with Dept of Social Development to reach more children under their care and maximize OVC support.
    • Support District facility visits

    Capacity Building

    • Support District to report on capacity building for paediatric and adolescent activities in the District (working with other Anova TAs and capacity building team)
    • Support other District managers on information related to paediatric and adolescent activities e., new information from province/national/conferences.

    Stakeholder engagement

    • Establish, manage, and maintain working relationships with province, district, sub-districts, facilities and other partner stakeholders for successful implementation of DOH and Anova workplans.
    • Working with other District managers, engage with paediatric team in the DCST and identify areas for collaboration and improvement for paediatric/adolescent HAST.
    • Working with other District managers, ensure paediatric and adolescent activities are included in Civil society platforms and report on SANAC NSP workplan activities.
    • In conjunction with Anova TAs, engage with NPOs for HTS and adherence support.
    • Attend monthly and adhoc meetings with supported sub-districts, district and partners and other departments.

    HR Manager

    The successful candidate will be required to provide HR partnership and leadership to the Head Office. The position requires a professional who is proactive in identifying resources and formulating actionable recommendations to support ongoing operations and program transitions.  They must be an articulate and persuasive communicator who is able to handle a broad range of HR issues in a fast paced and changing environment.

    Key duties and responsibilities:

    • Partner with line managers and provide comprehensive, pro-active, and holistic business partnering HR service to the District.
    • Assist in developing and implementing HR initiatives focused on achieving business objectives and improving the effectiveness of the HR department.
    • Manage the overall level of people risk in the portfolio and ensure that ER processes and procedures are adhered to.
    • Build relationships and network with line managers through effective communication.
    • Ensure effective and consistent implementation of the performance management process.
    • Generate monthly HR reports and action/ innovate for improvement on HR Metrics

    Minimum qualifications and experience:

    • Bachelor’s degree in at least one of the following disciplines: Human Resources, Labour Relations, Organizational Development, and Industrial Psychology
    • Minimum 5 years of HR Generalist experience
    • Minimum 5 years’ management and leadership experience
    • Experience working in an NPO/NGO highly advantageous
    • Demonstrated ability to creatively implement HR solutions and solve HR related problems
    • Ability to build strong working relationships across all levels of the organization
    • Proficiency in handling employee relations matters
    • Computer Literacy and proficiency in MS Office; SAGE 300 experience highly advantageous
    • Knowledge and experience working with BCEA, Employment Equity Act, Skills Development Act and Labour Law
