    Healthcare Coordinator

    Job Description

    • Faircape Health Tokai Estate is a Sub Acute Care and Rehabilitation Centre which provides restorative care and rehabilitation for patients recovering from a variety of medical conditions that include Neurological, Orthopaedic, Surgical, and Respite. At Tokai Healthcare, the focus is mainly on acute conditions that immediately affect patients' rehabilitation progress.
    • Our world-class care aims to bridge the gap between hospital and home. Our Multidisciplinary Team (MDT) of leading healthcare professionals administers care in the most therapeutic environment possible and supports healing outcomes in all aspects. 
    • The services of an experienced Healthcare Coordinator  (Day Shift) is required for our facility Tokai Estate in the Southern Suburbs. The successful incumbent is required to ensure the supervision of the Carer Team to facilitate practical, physical, social, and emotional care for residents and patients at the Healthcare Center:

    Duties include but are not limited to the following:

    Quality Control: 

    • Assist with weekly checklists 
    • Manage, allocate and oversee the carers on shift 
    • Ensure that all relevant and important information regarding the carers are handed over to Line Manager to ensure open communication
    • Efficient allocation of care staff 
    • Oversee that a high level of staff grooming is maintained
    • Effective reporting of maintenance issues and following up until resolved
    • Manage the discharge surveys and ensure it contains detailed information given by the patients
    • Maintaining the quality control standards 
    • Report non-compliance to procedures and ensure corrective methods are followed
    • Ensure patients complaints are reported to line manager immediately
    • Ensures high levels of customer service and creating a therapeutic and safe environment for our patients/residents

    Personnel and Human Resource:

    • Conduct daily briefings to discuss workflow, staff allocation, communicate expectations and to address any arising matters
    • Engage with carer staff to address performance and personnel problems and to discuss company policies.
    • Redirect findings of any performance issues to the line manager
    • Assist Manager with screening of potential job applicants and participate in the floor evaluation process
    • Manage staff probation meetings with the Line Manager 
    • Assist manager in evaluating employee performance and recommend personnel actions such as promotions, transfers and disciplinary actions.
    • Motivate staff to reach goals 
    • Orientate new staff members to the role and facility

    Administration: 

    • Manage the staff leave requests, sick day applications, work schedules and ensure allocations meet the demands of the facility
    • Ensure that staff break times are allocated fairly and done on a rotational basis
    • Ensure reports are completed and submitted to Line Manager
    • Attend meetings as requested and minutes taken and kept on record

    REQUIREMENTS:

    Education and qualifications:

    • Qualified Grade 12 
    • Tertiary education would be advantageous
    • Management or supervisory course would be beneficial

    Experience:

    • Proven Supervisory and Management 
    • Acknowledged Carer training course advantageous
    • Experience in a Healthcare setting advantageous
