    Case Officer - Dodrecht

    • Responsible for implementing and monitoring the Case Management Model (CMM) through adherence support for ART patients until they are eligible for decanting to any of the differentiated models of care.

    Minimum Requirements    

    • Grade 12 certificate
    • Enrolled nurse qualification will be an added advantage (most suitable for pediatric streams)
    • Able to communicate in English and isiXhosa is an advantage
    • At least two (2) years’ relevant experience in counselling and testing
    • Adherence counselling

    Duties and Responsibilities    

    • Capture details of clients on ART for less than one year and newly initiated patients on ART on the CMM excel spreadsheet, and plot all due appointments

    • Phone clients and conducts daily linkage to a CHW or any appropriate community person for those who missed their appointment; pay a visit to clients who missed their appointments, patients who are not virally suppressed, and patients needing additional care

    Data Capturer - Lusikisiki

    Purpose of the position:

    • To capture incoming clinical data on clients rendered service from the facilities, mobile and outreach teams; Maintain electronic database for data submitted; Check data correctness, completeness, consistency and data verification by liaising with relevant staff, conduct quality control of the study forms.

    Minimum Requirements    

    • Grade 12 / matric

    Duties and Responsibilities    

    • Capture quantitative and qualitative data into database and spreadsheets. 
    • Capture data collected in context of clinical service delivery into a specific database on daily basis and ensure safe keeping of client files and confidentiality (refer to POPI ACT). 
    • Assist with all data back-filling needs for study and program
    • Perform data quality checks and follow up with any discrepancies as appropriate
    • Refer to data quality principles (i.e. accurateness, completeness, timeliness, reliability & integrity of data) to conduct daily duties at all times.
    • Conduct quality control (QC) of study forms (i.e. verifications, calculate error rates)
    • Respond to queries relating to data as needed for the study and programme
    • Prepare/update forms and materials for study data collection
    • Conduct data mop up in all databases and populate reports. Accurately file study paper forms and store in a lockable cabinet in a designated office at the site.

    Facility Team Lead - Chris Hani

    Purpose of the position:

    • Responsible for overall facility performance and an anchor to ensure all program deliverables are successfully achieved through allocation of LOE to support QI, Case Management, Viral monitoring, CLI, eLabs and RTCQI etc.  Supports DoH in terms of CLI aspects at SPI-RT Level 3 and 4 facilities according to a routine schedule (Level 3 light touch support - each facility will have engagement minimum once a month; Level 4 surveillance support - each facility will have engagement minimum once a quarter; SEAD will provide technical assistance to QI mentors in this capacity; Level 2 and below facilities are directly supported by SEAD, the technical assistance partner for CLI).
    • The project aims to institutionalise and strengthen local capacity through provision of technical, mentorship and direct service delivery support to TB and HIV services when required and interacts with Ward Based PHC Outreach Teams (WBPHCOT) and Community Based Organisations (CBO) as required.         

    Minimum Requirements    

    • Bachelor’s Degree or Diploma in Nursing Sciences in health sciences
    • A diploma in Midwifery
    • A qualification in Primary Health care (PHC) or clinical assessment will be an added advantage
    • A diploma in HIV Clinical Management and /or NIMART

    Duties and Responsibilities    

    • Supervision of facility-based staff to ensure all program deliverables are fully achieved
    • Conducts skills audit exercise to identify training gaps.
    • Market CCMDD external pick-up points in communities
    • Focuses intensely on QI, Case Management, general CLI, viral monitoring through eLabs and suppressions, RTCQI,
    • Works collaboratively with Outreach Team Lead to ensure bidirectional referral, linkage and retention in care
    • Tracks facility and outreach performance against set TB and HIV clinical cascade indicators and advise accordingly on improvement strategies
    • Leads the development of QIPs working collaboratively with Facility Manager and Team to introduce change ideas followed by close monitoring of such intervention

    Nurse Clinician - East London

    Purpose of the position:

    • To provide a comprehensive community health service which includes HIV testing and counselling services linked to screening and diagnosing clients for TB/STIs with the TB contacts track & tracing. The provision of treatment, clinical care and case management to HIV & TB clients and ensure successful same day ART initiation with retention and adherence in care. For TB contact/s screening for TB, identify asymptomatic & symptomatic clients with same day start of TPT and completion of treatment.     Data Monitor - Durban

    Purpose of the position: 

    • To monitor and evaluate the data collected by the data capturers to ensure data integrity and accurate reporting.
