    Project Officer - Greater Letaba Rural

    • We are looking for a Project Officer for the Ward Based Primary Health Care Outreach Team (WBPHCOT) to supervise the Community Health Workers (CHWs) in the communities and link their duties to facilities, to implement the WBPHCOT program and strengthen it in communities and facilities. The incumbent/s will also do CHW capacity building, supervision and breach the gap between facilities and communities through the WBPHCOT program.

    Key duties and responsibilities:

    • Ensure implementation of the recent WBPHCOT guidelines and protocols
    • Collaborate with the WBPHCOTs and PHC managers to improve linkage of patients to care Orientation of the CHW's on tools, processes and practices of WBPHCOT
    • Facilitation of physical tracing
    • Support all Anova projects with planning, organizing and execution of support activities
    • Mentoring of staff at facilities in all program areas of WBPHCOT
    • Provide solution on complicated WBPHCOT cases together with clinics staff including referrals to relevant discipline at different levels of care
    • Monitoring of the WBPHCOT reports (DHIS and DORA) at facility and local area level
    • Timely submission of work plans, requests and reports
    • Assist CHW's with monthly and quarterly statistic collection and verification

    Minimum qualifications and experience:

    • Grade 12 (Matric)
    • Enrolled Nurse with more than 2 years PHC working experience
    • Registered with the South African Nursing Council with or without working
    • Experience on HIV and TB management
    • Good written and verbal communication skills
    • Proficiency in MS Office
